What to Expect During an In-Person Visit

  • All clients may be required to wear a mask or face covering while visiting Campbell Health Solutions. If you do not have a mask or face covering, one will be provided for you.
  • All providers will be wearing Personal Protective Equipment (PPE) during in-office visits, including masks, goggles/and or face shields per health department guidelines.
  • There will be limited access in all waiting room areas to support and maintain physical distancing.
  • Upon entering one of our clinics, a greeter will be asking pre-screening questions and providing masks or face coverings if needed.

We accept a variety of insurance plans, in addition to Medicare and Medicaid. All health care services are offered on a sliding-fee scale to those who are eligible. Don’t have insurance? Don’t worry, we can help.

As a Client Centered Medical Home, Campbell Health Solutions is committed to helping you stay healthy. We know that health is more than physical wellness; it also includes emotional and mental wellness.

Our health care team works to help you consider how your body, mind and behavior impact your health. You and our team members will set goals that will help to maintain and improve your health. Campbell Health Solutions does not offer traditional counseling or therapy. One of our team members might meet with you for 10-15 minutes during your visits to follow up on your goals. Our team member will give you information about the lifestyle changes recommended to improve your health and will help you decide how to work towards these changes.

Because Campbell Health Solutions is committed to your physical and mental health and wellness, routine medical treatment and procedures at Campbell Health Solutions may include:

  • A physical exam
  • Asking questions about your medical history and your physical/emotional health
  • Measuring your blood pressure, temperature, height, and weight
  • Prescribing or giving you medicine
  • Having blood drawn for tests
  • Setting health-focused goals
  • Other simple, common procedures
  • Routine behavioral health procedures may include asking questions about your mental health history and how you are feeling, discussing your concerns and problems, and creating a plan together.

All medical and health services are offered on a sliding-fee scale to those who are eligible. Most major insurance plans are accepted.

How Do I Apply?

In order to qualify you will need to meet the following requirements listed below.

  • Call 561-208-6595 to become a client.
  • Complete your application and proof of income.
  • If you are not able to print your application and documentation, please bring them to your next appointment.

Once all documents have been reviewed, you will be informed of the sliding fee based on gross annual income and household size. If you have questions about the sliding fee scale procedure, please contact 561-208-6595.

How Do I Qualify?

To qualify for the Campbell Health Solutions Sliding Fee Scale, you must bring proof of your “household” gross annual income. A “household” includes legal children, a civil union partner or married spouse, and legal dependents. Annual gross income includes salary, unemployment benefits, disability or social security benefits, investment income or other sources of income that support the household. You should bring proof of all of the household income for yourself and everyone in your household.

The following documents can be used for proof of income:

  • Most recent tax returns.
  • 2-3 most recent pay stubs from employer, no older than three months
  • Most recent W-2 or 1099
  • Letter from employer stating cash earnings that is signed by your employer or supervisor.
  • Letter from the Social Security Administration documenting your SSI or SSDI benefits.
  • Letter from the Social Security Administration documenting that you are ineligible for government benefits.
  • Letter detailing unemployment benefits from your state unemployment office.
  • Military Leave and earnings statement
  • Notarized letter stating you have no source of income signed by you.
