Linkage to Care and Case Management
In addition to our health care services, we also offer comprehensive Linkage to Care and Medical Case Management services to individuals living with HIV. Whether you’ve just been diagnosed or have been living with HIV for a while, we are here to help connect you to healthcare at Campbell Health Solutions as quickly as possible and provide you with support every step of the way. Most people living with HIV are able to see a medical provider within 72 hours of calling to schedule an appointment or being referred to Linkage to Care services. We’ll work with you to find out what your priorities are and help you develop an individualized plan to achieve your goals. We can help you coordinate your care and access other important resources, including:
- Mental Health Care
- Health Insurance
- Transportation Assistance
- Support Groups
- Medical Care Services
- Substance Use Treatment
- Housing Resources
- Employment Resources
- Dental Care
- Eye Care
- Legal Services (name change, immigration, etc.)

There is never a cost for Linkage to Care and Medical Case Management services.
Services for Individuals Living with HIV
We believe your healthcare should be designed with your unique needs and priorities in mind. That’s why we offer an array of services specifically designed to support the health and wellness of individuals living with HIV. All of our services are provided regardless of your insurance status. If you’re worried about not having insurance to pay for your HIV care, we’ll help you access other resources that will pay for your care, so you don’t have to. We’ll be with you every step of the way, so nothing stands between you and living well with HIV.
Medical Services
At Campbell Health Solutions, our clients are at the center of everything we do. We value and respect our clients just as they are, providing high-quality, non-judgmental care that affirms and celebrates every person who walks through our doors. Our medical providers are specially trained to care for the health of people living with HIV, in addition to their other healthcare needs. Whether you’re only coming in to see us to live well with HIV or you also have other healthcare priorities, our team will support you with all your health care needs.
Case Management
Case management can be critical in the effort to live well with HIV. A case manager will work with you to determine what your needs are and to find the best way to help you. Your case manager will link you to important support services and help find the answers to the complex questions about with living with HIV. They will be available to you on an on-going basis continually monitoring your needs and coordinating services. There is never a charge for case management services.
Here are some of the services that a case manager can provide:
- Assistance in applying for social security or public aid.
- Help in understanding complex private health insurance issues.
- Advocacy for public benefits and other services
- General financial assistance which can include emergency financial aid, food, housing, utilities, and transportation assistance.
- Referrals for medical, psychological, financial, legal, and other services.
